What this workshop registration form asks, and why
Each session is listed with its date and time as an option, so picking a session and a date is one decision instead of two. A second question asks how much the person has done before (first time, a little, fairly confident), which tells you how to pitch the room. Two optional boxes ask what they'd love to walk away with and anything you should know, such as access needs or allergies. Remove a session from the list once it's full, and add a new date if demand allows. The template works for classes, studio sessions and training days.

How to get good answers from a workshop registration form

Put dates in the options
Listing each session with its date and time saves a separate date question and avoids impossible combinations.

Read the goals before the day
Skim the 'walk away with' answers the evening before. A sentence aimed at what people asked for makes a workshop feel personal.

Mix the levels deliberately
If one session fills with beginners, steer confident people to another date so each group can move at its own pace.
