Inside the template

What this student registration form asks, and why

Three steps cover enrollment for schools, tutors and summer programs. The first collects the student's name, date of birth and email. The second asks whether the student is under 18; only then does it ask for a parent or guardian's name and email, so adult learners aren't asked for a guardian. Everyone gives an emergency contact number. The third step asks which course and any medical or accessibility needs staff should know about. Edit the course list to your own classes. Families can save the form and finish later, and a response limit closes enrollment when a course is full.

The 9 questions it asks

In order, with their field types and why each one is there. Pick one to see its screen above. Edit, reorder or delete any of them once it's yours.

  • The student's legal name, as it should appear on registers and certificates.

  • Date of birth, for age groups and records.

  • The student's own email, for timetables and course updates.

  • Under 18 or not. Only 'Yes, under 18' reveals the guardian questions.

  • Shown only for students under 18, so adult learners are not asked for a guardian.

  • Shown only for students under 18, the address for consent and school communications.

  • Asked of everyone, because adults need an emergency contact too.

  • The course or level. Edit the options to your own programs.

  • Optional, so staff can plan for medical or access needs before the first session.

2 questions are conditional. They only appear when an earlier answer calls for them, so most people answer 7, not 9.

Best practices

How to get good answers from a student registration form

  1. 1

Ask for guardians only when needed

Adult learners find a compulsory 'parent's name' field patronising. Showing it only for under-18s keeps the form right for both.

  2. 2

    Spread it over steps

Student, guardian and course details in three steps is easier to finish on a phone than one long page.

  3. 3

Ask about needs before the first day

Knowing about medical or access needs in advance means the first session goes smoothly for everyone.

  4. 4

    Set an enrollment deadline

Close the form a week before the course starts so registers, rooms and materials can be prepared.

After you publish

Where the responses go next

  1. Build the class lists

Export to CSV and sort by course to give each teacher their register.

  2. Keep the office informed

On Pro, send every registration to a Google Sheet or your student system through a webhook.

  3. Track numbers per course

The answer breakdown shows how many students chose each course while enrollment is open.

FAQ

Student Registration Form: common questions

Still deciding?

Seven steps from an empty account to a published form.

Are guardian details asked of every student?

No. Guardian name and email appear only when the student is under 18.

Can students save the form and finish later?

Yes. As a three-step form it has a 'Save & continue later' button, handy when a guardian's details need checking.

Can I add a consent checkbox?

Yes. Add a consent question and make it required, so a registration cannot be sent without it.

Can I take course fees?

Not through the form. FormSnapp has no payment field, so invoice or take fees through your own payment system.

Can I limit places?

Yes. Set a maximum number of responses and a 'course full' message.
