What this field trip permission slip asks, and why

It collects what a paper slip does and arrives when a parent submits it, instead of coming back creased in a school bag. Parents give the student's name and class, choose the trip, and add their own name, a phone number for the day and a second contact. Two yes-or-no questions cover medical or access needs and medication; each opens a detail box only when the answer is yes. A photo consent question follows, and the parent signs with a finger to give permission. List several trips in one form, or make a copy for each trip if you'd rather keep responses separate.

Best practices

How to get good answers from a field trip permission slip

  1. 1

Send it with the trip letter

Put the link or QR code in the trip letter or email, so parents can sign as soon as they read it.

  2. 2

Close it before the risk assessment

Set a closing date that leaves time to finalise numbers, medical plans and staffing ratios.

  3. 3

    Brief staff from the responses

Give every member of staff on the trip the medical and photo-consent list, not just the trip leader.

  4. 4

    Chase the missing slips early

Compare responses with the class list a week before and contact families who have not replied.

After you publish

Where the responses go next

  1. Make the trip list

Export to CSV and filter by trip in your spreadsheet for a list with contacts, medical needs and consent.

  2. Spot who needs medication

Search the responses for medication details to build the first-aid plan.

  3. Share the list with the trip leader

Export the signed slips to CSV the day before and give the trip leader a printed copy. On Pro, a private Google Sheet keeps it current.

FAQ

Field Trip Permission Slip: common questions

Still deciding?

Seven steps from an empty account to a published form.

Does it collect a real signature?

Yes. A drawn signature field, stored with the response and included in the export.

Do parents have to answer every medical question?

Only the ones that apply. Answering 'No' skips the detail boxes, so a parent with nothing to report is done in under a minute.

Can I run one slip for several trips?

Yes. The trip is a choice question; list your trips as options, or duplicate the form per trip to keep them separate.

Is the data stored securely?

Responses are stored in your FormSnapp workspace, available only to people you give access to. No IP address is stored with a response.

Can parents sign on a phone?

Yes. The signature field works with a finger on any touchscreen.
