What this petition form asks, and why

Supporters give their name and email, an optional postcode or town, and choose whether their name can be shown publicly or counted anonymously. An optional question asks why the issue matters to them, and those answers often make the best quotes for the campaign. They then sign with a finger or mouse and confirm the signature. Replace the welcome screen with your case: what you're asking for, who you're asking and what happens when enough people sign. Every form is rate-limited per IP address, and you can switch on a CAPTCHA if a petition starts attracting bots.
