Inside the template

What this donation form asks, and why

Preset amounts make giving a single tap, and 'Another amount' reveals a number field for anyone who wants to choose their own. Donors then pick once or every month, give their name and email, and can leave a message. Edit the amounts to your currency and to what each gift pays for. FormSnapp records the pledge but does not process payments, so the ending screen tells donors you will email them how to complete their gift. Send a payment link or direct debit form from your existing provider soon after each pledge arrives, while the donor's intent is fresh.

Best practices

How to get good answers from a donation form

  1. 1

    Say what each amount does

'£25 pays for a month of heating' gives a donor a reason. Put the impact in each option's label.

  2. 2

    Offer monthly giving

A small regular gift adds up to more than a one-off. Ask every donor once.

  3. 3

    Explain how payment works

The form records the pledge. Tell donors on the ending screen how and when the payment will be taken.

  4. 4

    Thank donors quickly

A personal thank-you within a day makes a second gift much more likely.

After you publish

Where the responses go next

  1. Take the payment

Reply to each pledge with a payment link, or direct-debit form for monthly donors.

  2. Keep a donor list

On Pro, send pledges to a Google Sheet or your donor CRM.

  3. See giving patterns

The answer breakdown shows which amounts and frequencies donors choose.
