What this membership form asks, and why
Three steps for clubs, gyms and associations. The first takes contact details. The second asks which membership (monthly, annual or lifetime in the sample), a start date, and how they heard about you, which shows whether existing members are bringing in new ones. The third collects an optional billing address, acceptance of your membership terms and code of conduct, and a drawn signature. Edit the plans to your own tiers.
