What this food order form asks, and why
Customers pick dishes from a menu laid out as a checklist, then note any allergies or things to leave out. They choose delivery or collection. Delivery orders are asked for an address; collection orders are asked what time they'll arrive, so nobody types an address they don't need. A last question records whether they'll pay by card or cash. That is a preference, not a payment, since FormSnapp doesn't take payments. Replace the sample menu with your own dishes, add prices to the labels, and close the form when the kitchen closes. A phone number is required, so you can call about anything unavailable.
