Inside the template

What this incident & safeguarding report asks, and why

The first question asks whether the reporter wants to say who they are. Name and contact fields appear only if they choose to, which reverses the usual order where identity is collected first. The form then asks what the concern is about, from bullying to worrying about someone else, and what happened, which is the only other required answer. Optional questions ask when and where it happened, whether it has happened before (with a box for earlier incidents), and what they'd like to happen now. The ending tells anyone in danger to find an adult immediately. Use it alongside your safeguarding policy, not instead of it.

The 10 questions it asks

In order, with their field types and why each one is there. Pick one to see its screen above. Edit, reorder or delete any of them once it's yours.

  • First, before anything else, so anonymity is a choice made before details are typed.

  • Shown only to people who choose to give their name.

  • Shown only to named reporters, so you can follow up.

  • What kind of concern it is, from bullying to worrying about someone else.

  • The only required detail, in the reporter's own words.

  • Optional, as precise as they can be.

  • Optional, which helps identify places that need supervision.

  • Whether this has happened before, with an option not to say.

  • Shown only when it has happened before, to learn about the pattern.

  • What the reporter wants to happen next, which shapes the first response.

3 questions are conditional. They only appear when an earlier answer calls for them, so most people answer 7, not 10.

University reporting

Add categories for housing and online harassment, and the student support contact on the ending screen.

Best practices

How to get good answers from an incident & safeguarding report

  1. 1

    Offer anonymity first

Asking for a name at the end, after the details are typed, is not a real choice. Ask at the start.

  2. 2

    Respond the same day

Have a named member of staff read every report on the day it arrives, and say so on the ending screen.

  3. 3

    Respect what they want

Some reporters want a conversation; some want it on record and nothing else. Start from their answer.

  4. 4

Tell them where to go now

Keep the line telling anyone in danger now to find an adult. A form is never the right channel for an emergency.

After you publish

Where the responses go next

  1. Route to one inbox

Create the form from the safeguarding lead's account, so reports land in their workspace and nobody else's.

  2. Limit who can see responses

Run reports from a workspace that only the safeguarding team can open. On Business, colleagues get their own viewer or editor role instead of a shared login.

  3. Watch for patterns

The answer breakdown shows which kinds of concern are rising, without exposing individual reports.

FAQ

Incident & Safeguarding Report: common questions

Still deciding?

Seven steps from an empty account to a published form.

Is it really anonymous?

If the reporter chooses not to give a name, no name or contact field is shown. FormSnapp does not store an IP address with a response.

Should this replace a safeguarding process?

No. It is a way in, not a policy. The ending says a member of the safeguarding team reads every report the same working day; keep that, and make it true.

Can we route it to one inbox?

Yes. Reports go to the account that owns the form. On Pro, a signed webhook can also pass them into a case system your team already uses.

Why is only one question required?

Someone reporting a concern should not be stopped by a field they cannot answer. Only 'What happened?' is required.

Can students fill it in on their phone?

Yes. It works on any device, and the link or QR code can be put on posters and the student portal.
