Inside the template

What this lead generation form asks, and why

A two-step form that qualifies a lead without feeling like an interrogation. Step one asks for a name, a work email and the company, which is optional because the email domain often answers it. Step two asks about team size, where they are in buying (just exploring, comparing options or budget approved) and what they're trying to solve. Only leads who say their budget is approved are asked when they want to start, so your most promising inquiries arrive with a timeline attached. Add a hidden field prefilled from your link to record which campaign or ad each lead came from.

The 7 questions it asks

In order, with their field types and why each one is there. Pick one to see its screen above. Edit, reorder or delete any of them once it's yours.

  • Who the lead is.

  • A work email, which identifies the company without a separate lookup.

  • Optional, since the email domain often says it anyway.

  • Team size, the quickest guide to which plan or package fits.

  • Where they are in buying. Only 'Budget approved' asks about timing.

  • Shown only to leads with budget, the people sales should call first.

  • What they are trying to solve, the opening line of the sales call.

1 question is conditional. It only appears when an earlier answer calls for it, so most people answer 6, not 7.

Best practices

How to get good answers from a lead generation form

  1. 1

    Qualify without interrogating

Ask only what changes your next action. Every other question costs leads.

  2. 2

    Keep the second step short

Nothing is saved until the form is sent, so a long qualifying step loses the whole lead. Three questions is plenty.

  3. 3

    Respond by stage

Call 'Budget approved' leads the same day, and send 'Just exploring' leads useful content instead of a sales call.

  4. 4

    Pass the source along

Add a hidden field filled from the link, such as the campaign name, so you know which ad brought each lead.

After you publish

Where the responses go next

  1. Send leads to your CRM

On Pro, a webhook, Zapier or Make step creates a contact in HubSpot, Pipedrive or your CRM.

  2. Alert sales

On Pro, post each lead to a sales Slack channel so hot leads are seen at once.

  3. Measure each step

On Pro, per-question drop-off shows whether the qualifying step is losing people.

FAQ

Lead Generation Form: common questions

Still deciding?

Seven steps from an empty account to a published form.

How does it qualify leads?

Team size and buying stage sort leads, and timing is asked only of those with an approved budget.

Can I track which campaign a lead came from?

Yes. Add a hidden field and fill it from the link, for example a UTM value, so it is saved with each lead.

Can I send leads to HubSpot or Salesforce?

On Pro, use Zapier, Make, n8n or a signed webhook to send each lead to most CRMs.

Why two steps?

Contact details first, qualifying questions second. Two short pages feel lighter than one long one, and the step indicator shows the end is close.

Can I embed it on a landing page?

Yes. Paste the embed snippet into any page or landing page builder.
