What this freelancer intake form asks, and why
Two steps for building a roster of freelancers and contractors. The first asks for a name, an email, a portfolio or website, which is required because work says more than a list of claims, and where they're based. The second asks what they do, from a searchable list of skills such as design, development, writing and video, how much capacity they have each week, and their day rate. A ballpark rate is enough, and asking early saves a conversation later. Search the responses by skill when a project comes in, and resend the form yearly to keep rates current.
